Major incidents: corrective actions for "never again"!

Industrial accidents will impact at least one of the following: worker safety, the environment, assets, future investments, reputation…and are always very disruptive!

What can be done to prevent it from ever happening again? After any type of incident, people are always greatly concerned and want to fix every distressing aspect. So where do you start?

    • Training, operating procedures, maintenance and best practices
    • Safety audit
    • Automation, controls and alarms
    • Emergency response program

    Incident investigation

    When dealing with incidents, BBA performs an investigation that clearly demonstrates the damaging path and answers the question, “How did we get here?” The analysis lists all causes, including the root cause.

    Risk ranking

    The most difficult task is then to evaluate the factors that led to the incident and to select which risk-reducing actions to implement. For example, is it better to train employees or to invest in safety automation? The answer is unique to your plant; there is no “one-size-fits-all” textbook solution!

    The right recipe

    The level of effort required to reduce the risk of another serious event is determined by weighing gains from individual solutions and gains from combined actions. Which option will provide the most for the effort?
